IntroductionSelective pulmonary venous gas (PV) analysis has been an important tool in evaluating donor lungs for transplant, providing objective support the clinical assessment at time of procurement. A low vein oxygen concentration a single isolated otherwise acceptable lung may predict early post-operative complications.Case ReportWe present 49-year-old male with end-stage disease secondary to sarcoidosis and hypertension. Donor arterial PO2 was 498, chest Xray demonstrated clear fields bilaterally without evidence significant opacity (Figure 1). flexible bronchoscopy revealed normal mucosa endoluminal lesions no secretions noted.
